Below are links to all the forms you need to have filled out and ready. We will have a notary public there to stamp all the forms that need that service (no charge). Please bring a photo copy of your current insurance cards, front and back; three small wallet sized photos of the players.
Players new to the Club must bring a copy of their Birth Certificate as well. Returning players do not have to bring a copy of the birth certificate.
Uniforms orders will be done online but we will have a session for trying them on another night as the final kit is not yet selected. We will also be doing a Club photo shoot session a little later where we will be getting the digital pictures we need of the players as well.
